How to Enroll
For your convience, we have three easy ways to enroll in the One Percent Club. In all instances, a member will contact you within several days of receipt to confirm your pledge.
- Online - Go to the "Enroll Now" online form, fill out the required information online, and click the "submit" button.
- Fax - Print out the "Enroll Now" form, fill it out, and fax it in.
- Mail - Print out the "Enroll Now" form, fill it out, and mail it in.
